How to Refer a Patient

Thank you for your interest in referring a patient to The Retina Group of Northeast Ohio. To refer a patient please call (330) 434-1185. To allow us to provide the best care for your patients please send a letter describing the reason for the referral and a summary of any diagnostic tests that have been performed. Our fax number is 330-922-9861.

What to Bring with You

On the day of your visit please bring the following with you:

  • Completed History Forms- if you were not given these forms in advance, they can be downloaded here.
  • Your insurance cards, copayment, and referral if required by your insurance company.
  • Photo Identification.
  • A driver to accompany you.

Appointments are subject to change in the event of an emergency. If the doctor is called away for an emergency we will attempt to inform you as soon as possible. You patience and understanding are greatly appreciated.

What to Expect

You should plan to spend approximately 2 hours at our office for an average new patient visit. This amount of time is necessary in order to do all of the examinations and diagnostic testing that is needed to form a treatment plan for your ocular condition.

Upon check in by our front office staff, an experienced ophthalmic technician will speak with you and take your medical and ocular history. They will then test your vision and dilate your pupils. Once your eyes are dilated the physician will examine you and discuss his exam finding. Various diagnostic tests and ocular testing may be recommended. Once the physician reviews your diagnostic tests, a treatment plan will be developed and discussed with you. If treatment is recommended, most treatments can be done in the office that same day.
